    Eisenhower Enrollment Form:  Please complete to give us more information about your student and to select your child's elective courses. 

     

    Records Request Form:  Please complete the top half of this form, so we can quickly order your child's records from their previous school.

     

    Free and Reduced Lunch/Breakfast:  Complete this form to see if you qualify for free and/or reduced lunch and breakfast at Eisenhower Middle School.  Please bring to registration with your other forms. 

    Verification of Residency:  Parents/Guardians should bring one item from this list, to prove residency in the Eisenhower School service area.
