Federal Tax Forms

    Everett Public Schools issues employees the following federal payroll tax forms:  Form W-2 Wage and Tax Statement, Form 1095-C Employer-Provided Health Insurance Offer and Coverage

    All federal tax forms will be posted to the district's employee online portal no later than the required deadline posted by the IRS for each form. Deadlines for the current tax year can be found on www.irs.gov.

    Instructions for accessing your form are as follows:

    For actively-employed staff (substitutes, coaches, games workers, etc.) - forms may be accessed through Employee Online. If you did not elect to receive your W-2 form electronically, the W-2 form will additionally be mailed to the address on file with Human Resources. If you were covered with health insurance through Everett Public School benefits at any time in the most recent completed tax year, your 1095-C form will be posted electronically only - it will not be mailed. After the required posting date determined annually by the IRS, if you would like a paper copy of your 1095-C form, you may request a paper copy by sending a request to payroll@everettsd.org. Requests will be completed within 30 days of receipt of the request. The 1095-C form is not required for individual income tax filing.

    For former staff (separated from employment in the previous tax year) or former employees and/or their dependents covered by PEBB - the W-2 Form will be mailed to the address on file with Human Resources. If you were covered with health insurance through Everett Public School benefits at any time in the most recent completed tax year, or through the Health Care Authority PEBB insurance program, your 1095-C form will be posted electronically only - it will not be mailed. After the required posting date determined annually by the IRS, if you would like a paper copy of your 1095-C form, you may request a paper copy by sending a request to payroll@everettsd.org. Requests will be completed within 30 days of receipt of the request. Please be prepared to authenticate your identity with any requests for these forms. The 1095-C form is not required for individual income tax filing.
